Does the world need another OB-wheel-due-date-calculator app? Youbet! OB Wheels combines three things that clinicians need to deriveall the dates necessary in pregnancy care, and no other app hasthis combination. It contains a calendar calculator to determineEDD (EDC) from LMP or from a sonogram report. The calendarcalculator also provides gestational age (EGA) as of any date for agiven EDD or LMP. Because the calculator works off the realcalendar, it is accurate for leap years too.Then for all of you whomiss your cardboard OB wheel, the app contains one which you canrotate, pinch and zoom, and generally play with just like a regularcardboard wheel. It's a little tricky to learn how to plant twofingers and rotate them rather than pinch-and-zoom to get rotationversus resizing. But the wheel works and it is accurate, especiallyso because you can zoom in to be sure where the little date hashlines lie.Finally, the app notes the most recent committee opinionscheme from ACOG on how one might reconcile due dates from LMP andsonograms to arrive at a single final EDD. To my knowledge, thescheme is authority-based and no better or worse than otherschemes, but my practice and my learners need a single clearscheme, so this app includes one for easy reference.As an educatorand clinician myself, I am grateful for feedback. Occasionally in medicine, we really need to "do the math". Thisappwill help. EBM Stats Calc let's a clinician do calculationsthatare hard or impossible to do in one's head. A clinician cangetNNT's (number needed to treat) from rates, percentages, orrawevent and patient numbers. And a clinician can use sensitivityandspecificity or likelihood ratios (LR+, LR-) to go from apretestprobability to posttest probability and positive predictivevalueand negative predictive value.Although these calculatorsthemselvesare somewhat novel for apps, the unique tool in this appis foreducators. Students and residents have a hard timeunderstandinghow the utility of a test can vary with pretestprobability.Instead of saying it in words and hoping they get it,now you candemonstrate it. A test might perform well, giving goodPPV and NPV,when pretest probability is in a certain range. Butslide thatpretest probability up or down and see the diagnosticutilitychange with the numbers flying right before your eyes. Thetoolalso shows a student that in many cases, an imprecise estimateofpretest probability doesn't much matter. Different doctorsmightjudge a clinical case to suggest likelihood of disease(pretestprobability) of 40%, 50%, or 60%.
